When signed the aforementioned agreement, Clients acknowledge that they have been provided with a copy of the Company’s Notice of Privacy Practices, which outlines how their PHI will be used and disclosed.

They have discussed the risks, benefits, and alternatives to hormone therapy with the Company’s medical team.
